Introduction: Benign prostatic hyperplasia (BHP) is one of the most common, progressive diseases in elderly men. It is very important to diagnose this disease in its early stages. The international prostate symptom score (IPSS) is a very helpful tool to be used for this purpose. Aims: The main goal of this study was to analyze the signs and symptoms of benign prostatic hyperplasia and its impact on a patient's quality of life. Patients and Methods: This descriptive randomized study included 112 male examinees aged 50-70 years. We used IPSS as a survey tool. Results: The average age of the examinees was 61.02 (± 6,18). 56.3% of examinees had mild symptoms, 31.3% had moderate symptoms and 12.5% of our examinees had severe lower urinary tract symptoms. Nocturia was the most common symptom. Irritative symptoms were present in 76.8% of examinees compared to obstructive symptoms which occurred in 8.9%, and 14.3% had both types of symptoms. Hypertension was the most common comorbidity (in 50.9% of cases). The majority of examinees answered that they would be satisfied with their quality of life if their symptoms do not change in the future. Conclusions: Assessment of symptoms and signs of benign prostatic hyperplasia using IPSS in clinical practice has proven to be useful in the timely approach of clinical physicians.

OBJECTIVE The hard palate participates in the construction of the oral and nasal cavities and represents the bony barrier between them. Morphological differences of the hard palate are important for forensic medicine, anthropology, anatomy, as well as scientific branches dealing with the study of evolutionary development, populations differences. The aim of this study was to determine sexual dimorphism of hard palate on three-dimensional (3D) models of human skulls using geometric morphometry. MATERIALS AND METHODS The research was conducted on 3D models of 209 human skulls from Bosnian population (139 male, 70 female). On the obtained 3D models, we marked landmarks on the palate using Landmark editor program. Using MorphoJ program we analyzed sex differences of shape and size on hard palate. RESULTS The principal component analysis showed that the first two components (PC1 and PC2) described 55.503% of the total morphological variability of the hard palate. The results of the discriminant analysis showed predictive power for male with 66.91% accuracy and for female with 58.57% accuracy based on the shape and size of the hard palate. The influence of size of the hard palate on its shape was statistically significant (p<0.0001). The results of discriminant analysis based on shape of hard palate showed predictive power for male with 68.34% accuracy and for female with 64.29% accuracy. CONCLUSIONS Sex differences of hard palate are statistically significant and can be used for sex determination in skeletal remains. The percentage of accuracy for determining sex based on the hard palate was higher for men in this study.

Background: The correlation between diabetes mellitus and acute myocardial infarction is greater every day. The mean platelet volume (MPV), which is the determinant of platelet function, is an independent risk factor for the cardiovascular disease. The aim of the study was to investigate the effect of each disease (hypothyroidism, hypertension, myocardial infarction) individually and combined on MPV in diabetic patients.Methods: The cross-sectional study included 102 patients who suffer from diabetes mellitus type 2 (DMT2), of both sexes (46 females, 56 males), with the average age of 58.91 (SD=12.93). All the patients were treated at the Primary Health Centre in Zenica from May to July 2017. All patients had diabetes mellitus and the disease had lasted for 10 years in both sexes.Results: Mean platelet volume was significantly higher in patients with myocardial infarction than in those without myocardial infarction. Age, sex, HbA1c, BMI, lipids and platelet count did not show any significance in either group of patients. Regression analysis showed that the prevalence of myocardial infarction had the highest predictive significance for MPV values, (predictor importance 0.49; coefficient 1.275, p<0.001).Conclusions: Mean platelet volume was significantly higher in patients with diabetes mellitus and myocardial infarction than in DM patients without myocardial infarction. Regression analysis showed correlation with acute myocardial infarction in patients with DMT2, but not with other chronic illnesses. The highest platelet volume indices were observed in patients with myocardial infarction. MPV can be used as a specific indicator in diabetic patients with myocardial infarction.   

Introduction: It is an undeniable fact that antidepressants can cause side effects. Antidepressants generally have a similar effect but they differ in their application safety, as well as their side effects. Aim: To determine differences in the frequency and intensity of antidepressant induced side effects in patients treated in primary care. Methods: The research was designed as a prospective, cross-sectional study, conducted on a voluntary and anonymous basis, and it included depression patients treated with antidepressant medications during 2013-2015 in Zenica-Doboj Canton using the Hamilton Depression Rating Scale and Toronto Side Effects Scale. Results: The total sample included 508 subjects. As a significant problem, abdominal pain was felt by 14% of subjects, indigestion by 19% of subjects, nausea by 15% of subjects, diarrhea by 9% of subjects, and constipation by 11% of subjects. 29% of subjects suffered from sweating, 20% suffered from a sudden heat stroke, 10% suffered from swelling, and 23% of them reported suffering from dry mouth as a significant problem. The prevalence of side effects in relation to how do they affect life and daily activities of subjects is statistically significant (P <0.000). Statistically significant side effects of SSRI antidepressants correlate with the duration of our subject’s treatment: perception of increased sleep (0.039) as well as decreased sleep (P = 0.009), sweating (P <0.001), sudden heat stroke (P <0.001), being without orgasm (P = 0.004), decreased libido (P <0.001), weight loss (P = 0.045). Conclusion: It is necessary to educate the patients about the nature and features of the depressive disorder, and to notify the patients of the expected course of recovery, as well as the need to adhere to the recommended therapy and the possible side effects of the medication.

Aim To determine the prevalence of depressive episodes and recurrent depressive disorders despite of the length of therapy and type of antidepressants. Methods The study was conducted among 508 patients aged 19-65 years who were treated for depression for at least 3 months (mild and moderate episodes were controlled and the effects of treatment monitored by family physicians, while severe episodes were controlled by a psychiatrist) during 2013- 2015 in Zenica-Doboj Canton using the Hamilton Depression Rating Scale (HDRS). Results The average age of the patients was 48.98±11.585 years. Depressive disorder was most commonly represented in patients with high-level education, 22%, more frequently in non-productive workers (non-productive vs. manufacturing 58%:35%). A significant number of patients who were treated for depression were unemployed (57%). All respondents were suffering from the most serious episode of depression with an average depression rate at the Hamilton scale 18.49±8.603, with a very serious depression level of 32%, severe 17%, moderate 21%, and mild 20%. Most patients were treated with paroxetine, 27%, fluoxetine 22% and sertraline 17%. Efficacy of depression treatment with different types of selective serotonin reuptake inhibitors antidepressants (SSRIs) was not significantly different (p=0.502). Conclusion Success of the treatment with the absence of symptoms of depression was achieved in 10% and the maintenance of depressive episodes occurred in 90% of cases. Adverse reactions with the most commonly prescribed SSRI in our country should be important in creating procedures and strategies for the future treatment of depression in family medicine.

Introduction: Due to sedentary lifestyles and excessive calorie intake, metabolic syndrome is becoming increasingly common health problem in the world, as well as in our country, and it is estimated to occur in 30% of the population of middle and older age. The metabolic syndrome is a combination of disorders that include: obesity, insulin resistance, glucose intolerance, impaired regulation of body fat and high blood pressure. Complications resulting from metabolic syndrome significantly reduces quality of patient’s life and represents a huge socio-economic burden. Metabolic syndrome therapy is directed to reduce all risk factors, and that means the change of lifestyle, which includes a reduction of body weight, physical activity, antiatherogenic diet and smoking cessation. Medical therapy is aimed to the individual risk factors. Case report: In case of our patient, despite the optimal standard therapy, including drugs for the regulation of LDL and HDL cholesterol and triglycerides, an intensive control of blood pressure and glucose, failure to implement the recommended treatment led to a myocardial infarction. Conclusion: The fundamental problem is not the lack of efficacy of available therapeutic measures, medications and procedures, but in insufficient implementation.

Aim of the study Exposure to radiation and aging are the leading causes of breast cancer among female patients. We aimed to investigate and assess the relationship between exposure to medical, diagnostic and iatrogenic radiation and breast cancer using a questionnaire among 100 newly diagnosed female breast cancer patients and 100 control female subjects without cancer. Material and methods A case control study using a family ambulatory based survey was conducted among 200 female patients from all municipalities of Zenica-Doboj Canton. New cases of breast cancer among subjects of experimental groups (n = 100) were diagnosed between 1 January 2003 and 31 December 2007 using the institutional clinical procedure for breast cancer diagnosis. Data were obtained using a self-rated questionnaire on radiation as a breast carcinogen. Data analysis was performed using SPSS version 19.0. Results There were no significant differences between the two groups and their subgroups for individual data and demographics except for prevalence of decreased family financial situation (practical poverty) among subjects with breast cancer in relation to control subjects (31%: 17% among control subjects; p = 0.001). Female patients who are exposed to iatrogenic radiation before the 3rd year of life (OR = 1.29; 95% CI: 0.839–1.985) and those who are exposed to CT more than twice per year are more than twice as likely to have breast carcinoma (OR = 2.02; 95% CI: 1.254–3.261) compared to control subjects. Poverty and low family income are vulnerability factors associated with elevated levels of breast carcinoma. This result is not in accordance with prior study results. ConcIusions It is necessary to develop an adequate registration system of iatrogenic exposure to radiation for each patient of any age, particularly for children aged < 3 years and for CT iatrogenic exposure.

AIM To analyse the difference in documentation of standard parameters for monitoring DM type 2 between Family Medicine Teams (FMT) and Teams on Program Additional Training (PAT). METHODS Study was conducted as 20 medical chart audits of diabetic type 2 patients randomly selected per 3 FMT from Zenica and 3 PAT from Kakanj. According to the chart, we assess sex, age, glucose in blood-GB, blood pressure-BP, total cholesterol-TC, body mass index-BMI, HbA1c, foot exam, eye exam and urinoanalisis and have any of the tests been done in the past year. RESULTS 60 medical chart from FMT and 60 medical charts from PAT teams were reviewed. FMT vs. PAT teams recorded: BG 58% vs. 30% (X2 = 8.651, p = 0.003); BP 70% vs.33% (X2 = 14.716, p = 0.0001); TC 35% vs.22% (X2 = 2.011, p = 0.156); BMI 48% vs.28% (X2 = 4.266, p = 0.038); HbA1c 41% vs.75% (X2 = 12.377, p = 0.0004); foot exam 26% vs. 78% (X2 = 28.158, p < 0.0001); eye exam 48% vs. 65% (X2 = 2.749, p = 0.097) and urinoanalisis 38% vs.88% (X2 = 30.179, p < 0.0001). CONCLUSION FMT recorded a higher number of metabolic parameters for macrovascular risk factors (BG, BP and BMI) than microvascular risk factors (HbA1c, foot exam and urinoanalisis) which are were better controlling by PAT teams.

Background: Numerous studies have observed risk factors for breast cancer. We investigated the association between quality life factors as breast cancer risks in a case-control study in industrial Zenica- Doboj Canton in Bosnia and Herzegovina. Methods: The case-control study was included 200 women, 100 without (control subjects) and 100 women with diagnosed breast cancer. We used questionnaires about breast cancer risks“ as study tool. Logistic regression was used to compute odds ratios (ORs) and 95% confidence intervals (CI) and a full assessment of confounding was included in analysis. Results: Breast cancer was positive associated with increasing age of life (from 45 years and more; OR= 1.25); further relative breast cancer history (OR= 4.42; 95%CI, 0.483-4.043); exposure to CT (OR=2.02; 95%CI, 1,254-3.261); never birth child (OR= 1.394; 95%CI, 0.808-2,407); used replacement hormonal therapy (OR= 1.826; 95%CI, 1.637-10.590); arrival time of menstruation (OR=2.651; 95%CI, 1.303-1.571); length of smoking status (OR=1.534; 95%CI, 0.756-3.098), alcohol consumption (OR=1.728; 95% CI, 0.396-7.533); exposure to CT per year (p=0.009), routine physical inactivity (p=0.009) and replacement hormones treatment (p=0.036). Conclusion: Inverse associations of breast cancer and poverty, arival time of menopause were observed. The link between breast cancer and a distant-cousin- degree family history of breast cancer was inverse association with breast cancer too. These results provide further evidence that, for most women, physical activity may reduce the risk of invasive breast cancer.
