Marine natural products represent a diverse collection of structurally distinct metabolites, many of which have untapped therapeutic potential. This study screened 161 marine-derived coumarin and xanthene compounds for their binding affinity to the histamine H₂ receptor and the gastric H⁺/K⁺-ATPase, the primary regulators of gastric acid secretion. Docking simulations were performed using curated structures of both targets, followed by an evaluation of the compounds for drug-likeness and predicted absorption, distribution, metabolism, and excretion (ADME) properties. Thirty-four compounds demonstrated a stronger predicted affinity for the H₂ receptor than famotidine; however, only three compounds (1, 5, and 150) met all drug-likeness criteria, achieving quantitative estimates of drug-likeness (QED) values exceeding 0.67. Screening against the proton pump yielded 98 hits with higher affinity than soraprazan, with compound 150 being the only candidate to fulfill all medicinal chemistry filters. Interaction analysis indicated that compound 150 binds to the proton pump in a manner that largely overlaps with soraprazan. Density functional theory (DFT) calculations were utilized to characterize the electronic properties of the most promising compounds. ADME predictions suggested favorable permeability and a low risk for human ether-à-go-go-related gene (hERG) inhibition, although high plasma protein binding and the potential for cytochrome P450 (CYP) inhibition may require further optimization. These findings underscore the potential of pyranocoumarin compound 150, along with xanthene derivatives 1 and 5, as promising candidates for the development of new acid-suppressive agents.

Abstract Parabens, often used as preservatives in consumer products, have raised concerns due to their endocrine-disrupting properties. The aim of this study was to quantify the levels of methyl and propyl paraben in adult urine samples and to assess potential health risks. Using high-performance liquid chromatography (HPLC), methyl and propyl parabens were detected in 20 participants at different concentrations. Methylparaben was more prevalent than propylparaben. Risk assessment was performed by calculating the estimated daily intake (EDI) and the hazard quotient (HQ), with HQ values indicating no significant health risk for the participants. Although current exposure levels appear to be safe, the long-term effects of chronic exposure remain uncertain, highlighting the need for further research. This preliminary study provides insight into paraben exposure in adults and contributes to the growing literature on the safety and prevalence of parabens.

OBJECTIVES The primary aim of this study is to detect and quantify the presence of Sildenafil (SDF) and Tadalafil (TDF) in dietary supplements marketed as natural sexual enhancers in Bosnia and Herzegovina. Additionally, the study seeks to utilize these findings to inform relevant authorities, enabling further testing in reference laboratories and prompting the necessary actions to remove these adulterated products from the market. METHODS Using high-performance liquid chromatography with diode array detection (HPLC-DAD), 20 samples were analysed for the PDE-5 inhibitors. RESULTS The analysis revealed that seven of the samples contained either SDF or TDF, with mean concentrations ± standard deviation (SD) ranging from 2,075.57 ± 0.47 µg/g to 33,808.857 ± 99.43 µg/g, and TDF concentrations ranging from 24.16 ± 0.11 µg/g to 3,994.66 ± 6.95 µg/g. CONCLUSION These findings indicate a significant health risk posed by the adulteration of these products. The widespread presence of these active pharmaceutical ingredients (APIs) in products falsely labelled as natural underscores the urgent need for stringent regulatory oversight and enhanced quality control measures to protect consumer safety. This study adds to the growing body of evidence concerning the adulteration of dietary supplements and emphasizes the critical importance of regulatory compliance and monitoring in safeguarding public health.

Abstract Dietary acrylamide exposure potentially poses health risks, including increased cancer risk and neurotoxic effects. There is no official data on acrylamide levels in food products on the Bosnia and Herzegovina (B&H) market, making it challenging to assess the associated health risks. As a non-EU country, B&H lacks national regulations aligned with Commission Regulation (EU) 2017/2158, which establishes benchmark levels and mitigation measures for acrylamide. This study used GC-MS to assess acrylamide content in fifteen food products from the B&H market, categorised as potato crisps/sticks, biscuits/wafers, and coffee. Acrylamide levels in some potato crisps and sticks, tea rings, and plain biscuits exceeded benchmark values, while levels in butter biscuits, biscuits with inclusions, filling or coating, wafers, and instant coffee, remained within acceptable limits. The highest acrylamide level was in potato sticks (1048.3 μg/kg), and the lowest in butter biscuits (23.8 μg/kg). Potato crisps/sticks had the highest average acrylamide levels (677.5 μg/kg), followed by tea rings and plain biscuits (444.5 μg/kg). Potato-based snacks accounted for the highest estimated dietary acrylamide intake. Most products exceeding benchmark levels originated from B&H, suggesting local producers might not fully apply mitigation strategies. These findings emphasise the need for regulatory reform, regular market monitoring, and targeted mitigation efforts.

Objectives: Food supplements and medicines which are not on the list of prohibited substances of the World Anti-Doping Agency are included in the group of permitted pharmacological agents for athlete’s recovery. The aim of this study was to describe qualitatively and quantitatively food supplements (FS) and over-the-counter drugs use among athletes in the last six month. Methods: This was a cross sectional study. Data on food supplements and the over-the-counter drugs, usage were collected during 2018 by self-administered, anonymous questionnaire. Results: A total of 112 athletes completed the survey. A total of 51.8% (n = 58) athletes reported the use of food supplements. The use of medical supplements was reported by 50.0% (n = 56) of athletes, 26.8% (n = 30) reported using ergogenic supplements, 1.8% (n = 2) using of sports food and 4.5% (n = 5) using other supplements. The use of over-the-counter drugs was reported by 35.7% (n = 40) of athletes. The over-the-counter analgesic drugs were used by 95% (n = 38) of over-the-counter drug users. Concomitant administration two or more over-the-counter drugs was reported by 40% (n = 16) athletes. Doctors and coaches had no advisory role in the use of food supplements or over-the-counter drugs.

Bu calismanin amaci, Bosna-Hersek'in farkli bolgelerinden farkli donemlere ait test sonuclarini kullanarak cig sutteki bu toksine maruz kalma ve saglik riski degerlendirmesi yapmaktir. AFM1 alim degerlendirmesi icin, Bosna-Hersek'te Tarim ve Kirsal Kalkinmaya Yonelik Katilim Oncesi Yardim Sektor Analizleri raporundaki veriler kullanilmistir. Aflatoksin M1'in cig sutteki konsantrasyonu icin, Saraybosna Eczacilik Universitesi Toksikolojik Kimya Fakultesi Laboratuvari'ndan alinan onceki calismalarin verileri ile yayinlanmamis veriler kullanilmistir. Dort maruziyet senaryosu icin yillik hepatoseluler karsinom riski (HCC) insidansi hesaplanarak risk karakterizasyonu yapilmistir. Sonuclar, ortalama tahmini gunluk aflatoksin M1 aliminin gunde 0.049 ng/kg vucut agirligi oldugunu gostermistir. Hepatoseluler karsinom riski (vaka/yil/100.000 kisi), HBsAg prevalansina bagli olarak nispeten dusuk bulunmustur (ortalama 0.000064-0.000074 vaka/yil/100.000). Son yillarda, aflatoksin M1'in kanserojen riskinin dusuk oldugu ve Bosna Hersek'te kritik olmadigi sonucuna varilmistir.

This work investigates the level of exposure to cadmium (Cd), mercury (Hg), and lead (Pb) via fish and seafood products in adult population in Bosnia and Herzegovina (BiH). Metals content was determined in seven commercial species of fish and seafood products widely available and consumed in BiH. Analysis of Cd and Pb was performed by GFAAS (Graphite furnace atomic absorption spectrometry), and analysis of Hg by FIAS AAS (flow injection cold vapour atomic absorption spectrometry) in accredited laboratory for food analysis. The health risk was determined by the estimated weekly intake, hazard index, target hazard quotients, and percent of tolerable weekly intake or percent of benchmark dose lower confidence limit. Concentration above the maximum residue level (MRL) set in the European Union was found in only one sample (Hg in mackerel). Cd content was generally high in squid, approaching the corresponding MRL in two samples. The hazard index was close to 1 in bluefin tuna and mackerel, mostly due to Hg content. These two fish species should be consumed in moderation, especially by pregnant women. While consumption of various fish and seafood on average is not of significant concern, health risk could not be ruled out for high consumers.

Background Polycystic ovary syndrome (PCOS) is a complex endocrinopathy affecting up to 20% of pre-menopausal women. The most recent international guidelines set lifestyle management as the cornerstone of the PCOS treatment. Still, there is a paucity of data on the implementation of lifestyle management in clinical practice. This cross-sectional study aimed to explore physicians-reported practices in PCOS lifestyle management in the Sarajevo Canton, Bosnia and Herzegovina (BiH). The profession of dietetics is not legally recognized in BiH. Nutritional interventions in health promotion and disease treatment are provided by medical professionals. Methods Data were collected by a paper-based questionnaire distributed during March-May 2018 in the Public Institution Health Centre of Sarajevo Canton. Results Forty-six physicians (response rate of 80.7%) completed the questionnaire. An initial treatment plan based solely on lifestyle measures (diet + physical activity), as recommended by current guidelines was reported by 34.8% of physicians. Although dietary interventions were rated as highly relevant in PCOS management by the vast majority of physicians, only one-half reported recording patients' adherence and 45.7% of physicians were unsure of the effectiveness of the lifestyle interventions in their patients. Conclusion PCOS lifestyle management in the study setting is sub-optimal. Additional education on effective PCOS lifestyle management strategies would be beneficial, especially for physicians with less than 15 years in practice. Possible obstacles to better physicians' engagement in PCOS lifestyle management should be further investigated.
