INTRODUCTION: Colonization with Pseudomonas aeruginosa (PA) is common in older kids with cystic fibrosis (CF) and there is increasing evidence that transmissible strains may cross-colonise patients. PA colonization is usually difficult to eradicate with antimicrobial therapy and, in some children and younger patients, infection is associated with rapid decline in pulmonary function and increased hospitalization. Diffuse bronchiectasis is seen in patients with cystic fibrosis. CASE REPORT: A 19-year-old male with CF severe lungs disease and liver insufficiency presented with coughing and breathing difficulties with wheezing progressed to generalized bad situations and Pseudomonas aerigenosa opportunic suprainfection. The effect of combined three antibiotics therapy was very successful as eradication of Pseudomonas aerigenosa. Lung physiological tests were pathologically changed with obstructive and restrictive reductions of vital pulmonal parameters. DISCUSSION: Cystic fibrosis (CF) is the most common potentially lethal genetic disease in the white population. Improvements in life expectancy have led to an increased recognition of lungs and hepatobiliary-pancreatic CF complications. CONCLUSIONS: Cystic fibrosis is a genetic disease usually diagnosed by abnormal sweat testing. Despite the best efforts by CF clinic pediatricians, patients with CF eventually reach the point of respiratory insufficiency, and lungs can no longer sustain, and patients need lungs transplantation.

Children's occult infections are characterised presenting pathogenic bacteries in blood of children in age 3 to 36 months, but they are good general aspect and orderly immunologic status and they don't have signs of focal infection. Manifestation of occult infections determined: age of child, increasing bodies temperature, testsphysical observance and clinical-biochemistry tests. Prevalence of manifestation occult infections is 3-8%, but they manifest ni a form occult bacteremia, occult pneumonia nad occult urinary infection. Methodic, systematic admission and adequate clinical-biochemical monitoring, we minimise sequeles of occult infections. Risk of serious sequeles at occult infections is importantly decreasing by epidemiological changes that it rises by using vaccination against Haemophilus influenzae and Streptococcus pneumoniae is leading ethiological source. Many contraversal opinions are presented in glance of therapeutic strategy at children's occult infection. Future of solutions at many hesitations ni context diagnosis and therapy of occult infections is established in using recent detectional tests /pneumococcus PCR, plasmas tumor reaction, interleukin lâ/ and preventive intervetions activities /conjugated pneumococcus vaccination/.

It is shown very rare case of eight years old girl with two secondary sicknesses: myeloic leucosis and adenocarcinoma of rectum that developed after six years of successful treatment. The diagnose of embryonal Rhabdomyosarcoma was set up in Sarajevo six years ago after surgical extirpation of tumor from orbitae and its pathohistology finding. Complete chemo and radiotherapy was performed in Germany (Bonn) according protocol (CWS 91). Diagnoses of acute myeloic leucosis and adenocarcinoma of rectum was set up according: clinical picture, peripheral blood smear, bone marrow smear and biopsy of bleeding polyp of rectum.

In retrospective study we analyzed 19 patients who were diagnosed and cured because of leukemia at Pediatric clinic in Sarajevo during last two years. From that number, 15 children (78.9%) were with acute lymphoblastic leukemia (ALL), 3 (15.7%) were with acute myeloic leukemia (AML) and 1 was with chronic myeloic leukemia. Among all pediatric malignancies percent of leukemias was 40.4%. Most of children with leukemia from Canton of Zenica-Doboj (42.1%) and then from Canton of Sarajevo (31.5%). Illness began mostly in winter months (36.5%), than in early spring (26.3%), and in autumn (26.3%), but there were no newly diagnosed in summer (July and August). More children with leukemia were from rural areas (57.8%), than from urban areas (42.2%).

The authors report the problem of central venous catheter appliance to the children with malignant diseases, employed for the first time in Bosnia and Herzegovina with the aim of pediatric oncologic patients treatment. During 1997 central venous catheter type Hickman was used in nine children between two and half to eleven years old (average six years and one months). The average time of catheter placement was six months, in two cases catheter were eliminated after two and three months respectively since application (spontaneous elimination and repeated septic attacks, caused bu resistant bacterial strains). Gram-positive bacteria have been isolated with eight children (Staphylococcus aureus and Staphylococcus epidermidis), and gram-negative enterobacteriaceae (Serratia marcescens, Pseudomonas aeruginosa, Klebsiella oxytocia and pneumoniae, Escherichia coli, Salmonella group C and Enterococcus faecalis) in samples taken from the catheter and hemoculture. The central venous catheter is useful in treating oncological patients, but may cause serious consequences, like local infections or septicaemia.

The authors have presented girl with supraventricular paroxismal tachyarrthymia and atipic WPW syndrome. The most frequently disrhythmias in childhood and at infants. Laboratories and others specific research are normal. SPT was proved with ECG. The Attacks SPT in girl are precipited usually nervous, fear, premenstruation phase or acute infection as in last hospitalization. In despite of many years medicament treatment attacks are repeated. The authors are presented diagnostics difficulties and advantages surgical intervention-ablation of A-V accesorius node (success 95%), after electrofiziologic investigations.

The case of Reiter's disease with eleven years old boy was reported in this paper. Thanks to anamenstic's data, clinical examination, laboratory findings and radiological evaluation we have established the diagnosis mentioned above. Joint's symptoms and signs prevail in clinical picture, and there are eye's disturbances and urinary symptoms. The patient was treated by antibiotics, uroantiseptics, steroids and non-steroids therapy too. Also, adequately phisiatric's treatment had a good influence to the course of this disease.
